Ce rapport résume les principales tendances épidémiologiques en 2022 pour 11 des maladies du calendrier vaccinal: la coqueluche, la diphtérie, les infections invasives à Haemophilus influenzae type b, à méningocoques et à pneumocoques, les oreillons, la poliomyélite, le rotavirus, la rougeole, la rubéole, et le tétanos. Des résultats plus détaillés par pathogène sont disponibles sur le site web de Sciensano (via la page des maladies à prévention vaccinale ou directement par agent pathogène via les liens dans le rapport).

Les vaccins introduits dans les programmes de vaccination belges ont considérablement changé l’épidémiologie des maladies infectieuses qu’ils ciblent, reduisant fortement leurs incidences. Certaines infections sont devenues sporadiques, comme le tétanos, ou ont été éliminées en Belgique, comme la rubéole congénitale et la poliomyélite. La vaccination et la surveillance de ces maladies restent néanmoins nécessaires en raison du risque d’importation et de la sévérité de ces maladies.

Dit rapport geeft een overzicht van de belangrijkste epidemiologische trends in 2022 voor elf van de ziekten uit het basisvaccinatieschema: bof, difterie, invasieve Haemophilus influenzae type b, invasieve meningokokkeninfecties, invasieve pneumokokkeninfecties, kinkhoest, mazelen, polio, rotavirus, rubella en tetanus. Meer gedetailleerde resultaten per ziektekiem zijn beschikbaar op de website van Sciensano (via de pagina vaccineerbare ziektes of rechtstreeks per ziektekiem via de links in het rapport).

De epidemiologie van de infectieziekten waartegen de vaccins gericht zijn, is sterk veranderd sinds de introductie van de Belgische vaccinatieprogramma's, met een belangrijke daling van de incidentie. Sommige infecties zijn sporadisch geworden, zoals tetanus, of werden in België geëlimineerd, zoals congenitale rubella en polio. Vaccinatie en surveillance blijven echter noodzakelijk vanwege het risico op invoer en vanwege de ernst van deze ziekten.

Since 2022, European countries have been facing an outbreak of mainly cutaneous diphtheria caused by toxigenic among asylum seekers. In Belgium, between 1 March and 31 December 2022, 25 cases of toxigenic infection were confirmed among asylum seekers, mostly among young males from Afghanistan. Multi-locus sequence typing showed that most isolates belonged to sequence types 574 or 377, similar to the majority of cases in other European countries. The investigation and management of the outbreak, with many asylum seekers without shelter, required adjustments to case finding, contact tracing and treatment procedures. A test-and-treat centre was organised by non-governmental organisations, the duration of the antimicrobial treatment was shortened to increase compliance, and isolation and contact tracing of cases was not possible. A vaccination centre was opened, and mobile vaccination campaigns were organised to vaccinate a maximum of asylum seekers. No more cases were detected between end December 2022 and May 2023. Unfortunately, though, three cases of respiratory diphtheria, including one death, were reported at the end of June 2023. To prevent future outbreaks, specific attention and sufficient resources should be allocated to this vulnerable population, in Belgium and at international level.

Sinds de aanbeveling van de Hoge Gezondheidsraad met betrekking tot de vaccinatie van zuigelingen tegen gastro-enteritis door het rotavirus en de invoering van de gedeeltelijke terugbetaling ervan in 2006, is een significante daling van het aantal bevestigde gevallen van rotavirusinfecties vastgesteld. Sinds 2012 lijkt de epidemiologie van het rotavirus te evolueren in tweejaarlijkse cycli waarin epidemiologische seizoenen met hoge en lage activiteit elkaar afwisselen.

In de seizoenen 2019-2020 en 2020-2021 is een heel andere trend waargenomen met een drastische daling van het aantal gevallen, meer bepaald 593 gevallen voor het seizoen 2019-2020 en 414 gevallen voor 2020-2021. Vóór 2020 telde een seizoen met lage intensiteit ongeveer 1500 gevallen en een seizoen met hoge intensiteit zo’n 2500 gevallen. De drastische daling is ongetwijfeld een rechtstreeks gevolg van de COVID-19-epidemie (het rotavirus circuleerde minder door de hygiënische maatregelen, lockdowns enz.).

De frequentste stammen in het seizoen 2019–2020 (genotypering uitgevoerd door het NRC) waren G2P[4] (27,6%, 26 gevallen), G9P[4] (27,6%, 26 gevallen), G9P[8] (12,7%, 12 gevallen), G3P[8] (11,7%, 11 gevallen) en G1P[8] (11,7%, 11 gevallen). Het aandeel van de gevallen in verband gebracht met G9P[4] was opvallend groot tijdens dit seizoen terwijl het vóór 2018 en na 2020 zelden werd vastgesteld. In het seizoen 2020-2021 hield een meerderheid van de gevallen verband met genotype G2P[4] (62,3% van de gevallen, 33 gevallen), gevolgd door G1P[8] (20,7%, 11 gevallen).

·        Invasieve infecties met Haemophilus influenzae type b (Hib) kennen een drastische daling sinds de invoering van de veralgemeende vaccinatie tegen Hib in 1993. Er zijn 10 invasieve infecties met Hib vastgesteld in 2019, 8 in 2020 en 3 in 2021.

·        Het Nationaal referentiecentrum (NRC) stelde 162 gevallen van invasieve infecties met H. influenzae vast in 2019, 84 in 2020 en 88 in 2021. Het lagere aantal gevallen in 2020 en 2021 is hoogstwaarschijnlijk het gevolg van de COVID-19-epidemie en de maatregelen die zijn genomen om de verspreiding van het virus tegen te gaan.

·        H. influenzae treft hoofdzakelijk 65-plussers en personen jonger dan 1 jaar. Onder de 65-plussers zijn er 32,0 gevallen/100.000 inwoners vastgesteld in 2019; 10,5 gevallen/100.000 inwoners in 2020 en 10,2 gevallen/100.000 inwoners in 2021. Onder de personen jonger dan 1 jaar zijn er in 2019, 2020 en 2021 respectievelijk 19,1 gevallen/100.000 inwoners; 11,1 gevallen/100.000 inwoners en 6,1 gevallen/100.000 inwoners waargenomen.

·        De meerderheid van de invasieve stammen van H. influenzae verzameld door het NRC was niet typeerbaar. Zij vertegenwoordigden 79,0% van de stammen in 2019; 73,8% van de stammen in 2020 en 78,0% van de stammen in 2021.

·        Er zijn 10 sterfgevallen gemeld aan het NRC in 2019 (1 gelinkt aan serotype F, 9 aan een niet-typeerbare stam), 4 in 2020 (4 niet-typeerbare stammen) en 4 in 2021 (1 gelinkt aan serotype F, 3 gelinkt aan een niet-typeerbare stam).

·       Les infections invasives à Haemophilus influenzae de type b (Hib) sont en nette régression depuis l’introduction de la vaccination anti-Hib généralisée en 1993. En 2019, 10 infections invasives à Hib ont été détectées, 8 en 2020 et 3 en 2021.

·       En 2019, Le Centre national de référence (CNR) a observé 162 cas d’infection invasive à H. influenzae, 84 en 2020 et 88 en 2021. Le nombre de cas moins important en 2020 et 2021 est très probablement lié à l’épidémie de covid-19 et aux mesures prises pour diminuer la propagation de ce virus.

·       H. influenzae touche principalement les personnes de plus de 65 ans ainsi que les moins d’un an. Ainsi pour les plus de 65 ans, en 2019, 32,0 cas/100.000 habitants ont été observés, 10,5 cas/100.000 habitants en 2020 et 10,2 cas/100.000 habitants en 2021. Pour les moins d’un an, 19,1 cas/100.000 habitants, 11,1 cas/100.000 habitants et 6,1 cas/100.000 habitants ont respectivement été observés en 2019, 2020 et 2021.

·       La plupart des souches invasives d’H.influenzae recueillies par le CNR étaient non-typables. Celles-ci représentaient 79,0% des souches en 2019, 73,8 % en 2020 et 78,0% en 2021.

·       Dix décès ont été signalés au CNR en 2019 (1 lié au sérotype F, 9 liés à une souche non-typable), 4 en 2020 (4 souches non-typables) et 4 en 2021 (1 lié au sérotype F, 3 liés à une souche non-typable).

Depuis la recommandation du Conseil supérieur de la santé concernant la vaccination du nourrisson contre les gastro-entérites à rotavirus et l’introduction de son remboursement partiel en 2006, une diminution significative du nombre de cas confirmés d’infection à rotavirus a été observée. Depuis 2012, l’épidémiologie du rotavirus semble évoluer en cycles bisannuels, avec une alternance entre saisons épidémiologiques d’activité intense et saisons d’activité faible.

Pour les saisons 2019-2020 et 2020-2021, une tendance toute autre a été observée avec une baisse drastique du nombre de cas, c’est-à-dire 593 cas pour la saison 2019-2020 et 414 cas pour 2020-2021. Avant 2020, le nombre de cas par saison d’intensité faible était plutôt d’environ 1500 cas et pour les saisons d’activité intense d’environ 2500 cas. La baisse drastique est sans doute une conséquence directe de l’épidémie de covid-19 (moindre circulation du rotavirus suite aux gestes barrières, confinements, etc.).

Pour la saison 2019-2020, les souches les plus fréquentes étaient le G2P[4] (27,6%, 26 cas), le G9P[4] (27,6%, 26 cas), le G9P[8] (12,7%, 12 cas), le G3P[8] (11,7%, 11 cas) et le G1P[8] (11,7%, 11 cas). La proportion de cas liés au G9P[4] était remarquablement élevée durant cette saison, alors qu’il était rarement observé avant 2018 et après 2020. Pour la saison 2020-2021, une majorité des cas étaient liés au génotype G2P[4] (62,3% des cas, 33 cas), suivi par le G1P[8] (20,7%, 11 cas).

Background

Legionnaires’ disease (LD) is a severe bacterial infection causing pneumonia. Surveillance commonly underestimates the true incidence as not all cases are laboratory confirmed and reported to public health authorities. The aim of this study was to present indicators for the impact of LD in Belgium between 2013 and 2017 and to estimate its true burden in the Belgian population in 2017, the most recent year for which the necessary data were available.

Methods

Belgian hospital discharge data, data from three infectious disease surveillance systems (mandatory notification, sentinel laboratories and the national reference center), information on reimbursed diagnostic tests from the Belgian National Institute for Health and Disability Insurance and mortality data from the Belgian statistical office were used. To arrive at an estimate of the total number of symptomatic cases in Belgium, we defined a surveillance pyramid and estimated a multiplication factor to account for LD cases not captured by surveillance. The multiplication factor was then applied to the pooled number of LD cases reported by the three surveillance systems. This estimate was the basis for our hazard- and incidence-based Disability-Adjusted Life Years (DALYs) calculation. To account for uncertainty in the estimations of the DALYs and the true incidence, we used Monte Carlo simulations with 10,000 iterations.

Results

We found an average of 184 LD cases reported by Belgian hospitals annually (2013–2017), the majority of which were male (72%). The surveillance databases reported 215 LD cases per year on average, 11% of which were fatal within 90 days after diagnosis. The estimation of the true incidence in the community yielded 2674 (95% Uncertainty Interval [UI]: 2425–2965) cases in 2017. LD caused 3.05 DALYs per case (95%UI: 1.67–4.65) and 8147 (95%UI: 4453–12,426) total DALYs in Belgium in 2017, which corresponds to 71.96 (95%UI: 39.33–109.75) DALYs per 100,000 persons.

Conclusions

This analysis revealed a considerable burden of LD in Belgium that is vastly underestimated by surveillance data. Comparison with other European DALY estimates underlines the impact of the used data sources and methodological approaches on burden estimates, illustrating that national burden of disease studies remain essential.

Acute gastroenteritis (AGE) remains a common condition in both low- and high-income countries. In Belgium, however, there is currently a lack of information on the societal health and economic impact of AGE. We conducted a retrospective study using mortality and cause-of-death data, hospital data, primary care data, health interview survey data and other published data. We estimated the burden of illness during a 5-year period (2010-2014) in Belgium in terms of deaths, patients admitted to hospitals, patients consulting their general practitioner (GP) and cases occurring in the community. We further quantified the health impact in terms of disability-adjusted life years (DALYs) and the economic impact in terms of cost-of-illness estimates. We estimated 343 deaths, 27 707 hospitalised patients, 464 222 GP consultations and 10 058 741 episodes occurring in the community (0.91 cases/person) on average per year. AGE was associated with 11 855 DALYs per year (107 DALY per 100 000 persons). The economic burden was estimated to represent direct costs of €112 million, indirect costs of €927 million (90% of the total costs) and an average total cost of €103 per case and €94 per person. AGE results in a substantial health and economic impact in Belgium, justifying continued mitigation efforts.

Background: The Haemolytic Uremic Syndrome (HUS) is the most severe manifestation of infection with Shiga toxin-producing (STEC). In Belgium, the surveillance of paediatric HUS cases is conducted by a sentinel surveillance network of paediatricians called Pedisurv. In this article, we present the main findings of this surveillance from 2009 to 2015 and we describe an annual incidence of HUS.

Methods: For each case of HUS <  15 years notified by the paediatricians, clinical, microbiological and epidemiological data were collected by a questionnaire. National hospital discharge data with ICD-9 code 283.11 were used to calculate the incidence of HUS in children < 15 years.

Results: From 2009 to 2015, 110 cases were notified to the Pedisurv network with a mean annual notification rate of 0.8/100,000 in children < 15 years. Death occurred in 2.5% of all patients and the median number of days of hospitalization was 10 days. One third (35.4%) of the HUS cases were confirmed positive STEC, with a majority of STEC O157. The mean annual incidence based on the hospital discharge data was 3.2/100,000 in children < 15 years and 4.5/100,000 in children < 5 years.

Conclusion: The incidence of paediatric HUS in Belgium is high compared to other European countries. Its surveillance in Belgium is quite comprehensive and, although less effective than monitoring all STEC infections to detect the emergence of outbreaks, is important to better monitor circulation of the most pathogenic STEC strains. In this context, efforts are still needed to send samples and STEC strains from HUS cases to the National Reference Centre.
