Description of the test

Determination of (1→3)-β-D-glucan in serum and CSF (Wako method)

Purpose of test

(1→3)-β-D-glucan is released into the serum in various invasive fungal infections including invasive Candida infections, invasive aspergillosis and Pneumocystis jirovecii pneumonia (but NOT in invasive mucormycosis or cryptococcosis) and is thus regarded as a “panfungal marker. 

The main asset of the test is its high negative predictive value which allows a negative result to be the trigger to discontinue antifungal (or antibacterial in case of PJP) therapy. 

Criteria for performing this test in the context of reference activities

  • Strong suspicion of invasive fungal infection where other diagnostic tests are negative or inconclusive.
  • Suspicion of Pneumocystis jirovecii pneumonia in a patient where it is not possible to perform bronchoalveolar lavage. 

Instructions for samples

Test is performed on serum and CSF.
Min 20 µL of serum and CSF required.

  • Shipping: refrigerated (2-8°C).
  • If delayed in shipment, freeze sample and send frozen.
