Consumption of vitamin D

Why do we study the consumption of vitamin D supplements?

  • Vitamin D is a fat-soluble vitamin essential for the absorption of calcium from the diet. It plays a key role in the growth and maintenance of strong bones and teeth. In addition, vitamin D is important for proper muscle function and the immune system. The most important source of vitamin D is sunlight exposure, which triggers its synthesis in the skin. However, due to Belgium’s northern latitude, sunlight exposure is often insufficient, particularly in the winter months, limiting the body’s ability to produce adequate vitamin D. This increases the risk of deficiency, which can lead to conditions like osteomalacia and osteoporosis. 
  • While vitamin D can also be obtained from dietary sources such as fatty fish, fortified dairy products, and eggs, these alone often do not provide enough to meet daily needs. To address this, the Belgian Superior Health Council recommends vitamin D supplementation: 10 µg/day for children and adults, 15 µg/day for those at risk of osteoporosis, and 20 µg/day for pregnant, breastfeeding women, and the elderly. These recommendations may vary depending on individual sun exposure, with lower doses advised for those with more sunlight exposure.
  • Despite the benefits of supplementation, overconsumption can be harmful. Vitamin D supplements are widely available over the counter in Belgium, raising concerns about potential overuse.  Therefore, it is essential to monitor supplement intake to ensure both adequacy and safety.

 

How do we study it?

  • Participants (or the parents of the 3-9 year old children) were asked to complete a Frequency Questionnaire on supplement consumption. They reported whether they had used a supplement in the past 12 month.  Supplement users were then asked which type of supplement they had taken and during which months. To analyse seasonal differences, the year was further divided in winter (December-March) and the rest of the year (April-November).
  • The brand name and dose of those who indicated using vitamin D supplements was asked to get a more complete picture of Vitamin D supplement use.  The use of dietary supplements was monitored during the two 24-hour dietary recalls. Participants were asked whether they had taken any supplements on the previous day. If so, the interviewer recorded the brand and product name of the supplement.    
  • Note that this was a self-reported  questionnaire, the answers were given by the participant based on their memory and their knowledge of the supplement type. Therefore, results should be interpreted with caution.

The use of vitamin D supplements increased with 9% since 2014-2015,  from 19% to 28%
Vitamin D supplement use is highest among women aged 65 and above
Overall, less men use vitamin D supplements than women
34% of vitamin D users take the highest dose of 25000 IU* (=625µg) per supplement

  • Older adults (65 years and above) do not include those residing in care facilities, hospitalised individuals, or those who would have required significant assistance during interviews (e.g., people with cognitive impairments).  
  • Across all the age categories, women are more likely than men to consume vitamin D supplements. The largest difference is observed among older adults (aged 65 and above), where 30% of men use a vitamin D supplements compared to 48% of women.  

By educational level

  • Crude = results weighted for season, age, sex, and socioeconomic status.
  • The educational level is calculated at the household level by considering the highest diploma obtained by: (i) the respondents or their partner for adults (aged 18 years and above), or (ii) the father and mother (or carer, if applicable) for children and adolescents (aged 3 to 17 years). However, for simplicity, we refer to the educational level of individuals rather than the educational level of their households.
  • People with a high education level indicated the highest use of vitamin D supplements (35%) in comparison to people with a lower education level (28%).   
  • Vitamin D is used by 32% of people with mid educational level.

By region

  • Brussels is excluded from the regional comparison due to an insufficient number of participants, but is included in the national data for Belgium.
  • In Wallonia 37% of the population uses a vitamin D supplement, compared to 27% of the population in Flanders.

  • Adjusted = results weighted for season, age, sex, and socioeconomic status, and adjusted according to age and sex based on linear regression model (using the Belgian population of 2022-2023 as reference). 
  • Adults aged 65 years and older are not included as they were not part of the target population of the 2014-2015 survey.
  • The use of vitamin D supplements has increased with 9%, from 19% in 2014-2015 to 28% in 2022-2023.

  • 34% of vitamin D supplement users, takes the highest dose of 25000IU (=625 µg), while 14% of users takes a dose 800 IU (=20µg) and 11% takes a dose of 1000 IU (=25µg). 
  • 59% of the consumers of the highest dose (25000IU)takes this dose one to three times a month and 25% takes it once per week.   
  • 74% of the consumers of the 1000IU dose take this on a daily basis. 
  • IU= international unit : 1 IU= 0,025µg
